What do you need for your appointment

Please remember to:

  • Bring your valid Medicare | DVA | Seniors Card

  • Bring a list of any medications you are taking

  • New patients, please print and fill in our new patient form – New patient form or;

  • Arrive 10 minutes prior to your appointment to fill out the necessary paperwork.

  • To avoid a cancellation fee of $50 please contact us 3 hours in advance prior to your appointment. 

  • Allied Health cancellation fees differ from general practice fees. Please consult your allied health professional for further information regarding no show and late cancellation fees

We’ll make every effort to book you in for an appointment with your preferred doctor and time. If we can’t offer you your first choice, we’ll find the next best appointment available. If you’re unable to attend your appointment, please contact us and give as much notice as possible.

RACGP

The majority of our GP’s have undertaken additional specialist training and have been awarded The Fellowship of the Royal Australian College of General Practitioners (FRACGP). This is a specialist general practice qualification accredited by the Australian Medical Council.

Some consultations will incur a fee including:

  • Pre Employment Medical

  • Commercial/Heavy Vehicle Licences

  • Procedures

  • Drive Assessment

Bulk Billing for Concession Patients

Yarra Glen Family Clinic recognises the need to provide access to subsidised medical services for the following patients:

​

– Patients who hold a valid Health Care Card, Pensioner Concession Card, Commonwealth Seniors Health Card or DVA Gold Card

– Children who are under 16 years of age.

​

For those who hold the above mentioned cards, or are under 16 years of age, we are pleased to confirm that there will be no gap fee for standard GP consultations during ordinary hours.

Medical Procedures

Medical procedures are privately billed for all patients. Please inquire about the cost of your procedure at the time of booking.

Nurse Visits

All nurse appointments will incur a fee. Please inquire about these fee's with our reception team, before your appointment.

On the Spot Medicare rebates

Our friendly Reception team can assist you to claim your Medicare rebate on the spot. A physical debit card will be needed to do this. Alternatively our team can advise ways to claim your rebate back yourself.

If you have any concerns regarding the care provided to you by Yarra Glen Family Clinic, please contact our Practice Manager or discuss the issues with your GP.

You may prefer to e-mail through your concerns which is more than welcome. Contact our reception staff today for the practice manager's e-mail address. However, if you feel the need to discuss this outside of our practice please contact
